The chart here shows you how the
assessment is organised into sections.
There are four main sections – Program Design and Planning;
Program Evaluation; Organisational Governance and Management; and
Program Operations.
Answer the questions in each section by clicking the rating that
is most relevant to your program.
The assessment is simple with a 0-5 ranking. 0 being not applicable.
1 being rarely and 5 being always.

If you need to stop and save your work, click Save and Continue
Later.
You can return in the future, using your user name and password to
login and complete the assessment. When you return, click on View My Reports to
access your incomplete reports.
When you have answered all the questions, click Submit Assessment at the
bottom of the page.
Your reports will show the date the assessment was completed and
will be given a reference number.
If you want a hard copy, click the “Print” icon to print
your full report
You may also email the PDF file of your full report to a friend or
colleague.
To download, click the “Download Report - PDF format ” to
download your individual or full reports.
You can return at any time and login to view your reports online.
You can complete the self assesment tool as many times as you wish.
Each report will appear separately with a different assesment number.

you will need to use Adobe Acrobat. Click

have it on your computer.
We recommend that the report is done at least twice
a year to monitor progress.
