            If you can obtain direct consent from the individuals for us that would be the 

            best option. Simply use the Media Consent Form – Individuals (PDF, 75.1kb)

            If you have already sought consent for previous images/footage and are

            passing on this consent to the Network use the

            Media Consent Form – Mentoring Organisations (PDF, 73.9kb)

Step 3: Email or post the information along with the photos/video or audio files to the
