Submitting your Mentoring Stories
Step 1: Download a Mentoring Story Form (word doc, 120kb) and Complete
Step 2: Download the relevant Media Consent Forum If you can obtain direct consent from the individuals for us that would be the
best option. Simply use the Media Consent Form – Individuals (PDF, 53.1kb)
If you have already sought consent for previous images/footage and are passing on this consent to the Network use the
Media Consent Form – Mentoring Organisations (PDF, 51.6kb)
Step 3: Email or post the information along with the photos/video or audio files to the AYMN. Email: info@youthmentoring.org.au or post to: GPO Box 10500 Sydney
